Due to the state of medical technology before the 1920s, most people had very low medical expenses. The main cost associated with illness during this time was lost wages from work. On account of this, most people most people bought "sickness" insurance to get income in case of illness.
In addition to the low demand for health insurance, commercial insurance companies were reluctant to offer private health insurance policies. Commercial insurance companies did not consider people's health an insurable commodity. Generally, people felt individual health insurance was unnecessary before the 1920s; therefore, there was no nationalized health insurance. Although many European nations had some sort of compulsory health insurance plan in place, America did not for the following reasons:
Support for legislation was low due to lack of demand
Pharmacists and Physicians opposed legislation because they feared it would undermine their business
